Le représentant ou la représentante des soins aux patients doit faciliter les traitements de perfusion pour les patients. Le titulaire de ce poste connaît les objectifs de la division et les attentes relatives aux IRC. Les responsabilités concernant les cliniques qui lui sont attribuées comprennent les directives médicales pour les patients; garantir la disponibilité de personnel qualifié durant les traitements de perfusion, et confirmer l’accès aux fournitures pharmaceutiques et médicales d’urgence. Les traitements médicaux seront planifiés conformément aux contrats de niveaux de service. Le représentant des soins aux patients offrira un service à la clientèle hors pair à tous les clients, notamment aux patients, aux médecins, au personnel des cliniques et aux représentants pharmaceutiques.

Responsabilités

  • Planifier les rendez-vous des patients pour les perfusions selon les régimes de traitement des directives médicales déterminées par les médecins prescripteurs.
  • Offrir des services de prise de rendez-vous aux patients et aux infirmières en dehors des heures normales (soirs et fins de semaine), comme l’exigent les besoins des cliniques et la direction d’INVIVA.
  • Vérifier que les directives médicales contiennent toute l’information pertinente et qu’elles ont été signées par le médecin prescripteur.
  • Vérifier que les patients sont traités dans le respect des directives médicales prescrites.
  • Documenter les données du registre de suivi conformément aux directives.
  • Effectuer la coordination du personnel qualifié et des médecins pour garantir des soins adéquats durant les perfusions des patients.
  • En collaboration avec les coordonnateurs des remboursements ou les pharmacies, offrir un soutien si nécessaire afin de déterminer l’état du remboursement du patient avant de planifier des rendez-vous.
  • Vérifier que les sites de perfusion disposent des traitements et des fournitures d’urgence recommandés.
  • Fournir aux médecins prescripteurs un rapport de rétroaction sur la perfusion dans les 48 heures suivant cette dernière.
  • Effectuer un suivi de la date limite de validité des directives médicales et communiquer avec les médecins traitants pour fournir des directives médicales à jour.
  • Signaler tout événement indésirable, indépendamment de sa gravité ou de son lien ou absence de lien à un médicament.
  • Aider l’équipe afin d’assurer la fluidité du service fourni par tous les coordonnateurs de perfusion.
  • Comprendre les IRC des divisions et chaque contrat de niveau de service pour traitement médical.
  • Représenter Mckesson lors de réunions, de congrès et d’autres événements professionnels.

  • Excellentes compétences informatiques, dont une connaissance de Microsoft Office.
  • Capacité à cerner les principaux enjeux; surmonter les défis ou les obstacles internes de façon créative.
  • Intégrité et discrétion absolues, respect de l’obligation de rendre des comptes.
  • Solides compétences en matière de planification et d’exécution, capacité à réfléchir et à agir rapidement.
  • Permis de conduire valide répondant aux exigences liées aux déplacements.
  • Connaissance fonctionnelle des exigences commerciales ou réglementaires qui concernent cette industrie.

**This is a 6 month contract

The Patient Care Representative is responsible for facilitating infusion treatments for patients. The Patient Care Representative will be knowledgeable regarding the division's goals and KPI expectations.  Responsibilities for their assigned clinics will include medical directives for patients; ensuring qualified personnel are available during the infusion treatments, confirming that the pharmaceutical and emergency medical supplies are available. Medical therapies will be scheduled as per SLA’s.    The Patient Care Representative will provide excellent customer service to all clients including patients, physicians, clinic personnel, and pharmacy representatives.

Responsibilities:

  • Scheduling referred patients for infusions in accordance with the treatment schedules in medical directives established by the prescribing physicians;
  • Ensure medical directives have all pertinent information and have been signed by the prescribing physician;
  • Ensure that patients are treated in accordance with prescribed medical directives;
  • Document tracking log data as per guidelines
  • Provide coordination of qualified personnel and physicians to ensure adequate care during patient infusions;
  • Coordinate, as required, with reimbursement coordinators and/or pharmacies to determine patient’s reimbursement status prior to scheduling appointments;
  • Ensure infusion sites have recommended emergency medications and supplies;
  • Provide prescribing physicians with an infusion feedback report within 48 hours of the infusion performed;
  • Keep track of the expiry date on medical directives and contact the referring physicians to provide current medical directives;
  • Report any adverse event, serious or non serious, whether or not it is deemed drug related;
  • Assist team to ensure a continuum of service provided by all infusion coordinators;
  • Ensure an understanding of the divisions KPI and each medical therapy SLA
  • Represent McKesson Infusion Services in meetings, conferences and other business functions.

Additional Qualifications:  

  • Post-secondary diploma / degree in customer service or administration is a definite asset.
  • Experience in coordinating client/patient services.
  • Minimum of 2 years customer service experience (public and/or private).
  • Experience in managing high volume client enquiries.
  • Demonstrated superior organization and communication skills.
  • Computer literacy (MS Office, internet).
  • Fluent in English spoken and written.
  • Fluent in French is an asset.
  • Clinic experience is an asset
  • Medical terminology is an asset

The pay range shown below is aligned with McKesson's pay philosophy, and pay will always be compliant with any applicable regulations.
