CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families live in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.

Case Management Supportive Services (CMSS) provides community-based HIV/AIDS case management, health education/risk reduction and the provision of food voucher services to individuals People Living with HIV/AIDS (PLWHA), that have income below 500% of Federal Poverty Level based on household size and are New York State residents. This dynamic model of case management, health education/risk reduction and food voucher services will actively address the multiple needs of PLWHA and remove the barriers that prevent retention in care and treatment. This case management model serves as a bridge to help consumers learn to navigate the health care and support system and gain the knowledge and skills necessary to effectively self-manage their care. It also helps support consumer entry into the full continuum of care that is available. Case management efforts must focus on improving health outcomes, leading to viral suppression.

SPH-1: The SPH 1 Program provides twenty units of permanent Scattered-Site apartments to single adults living with HIV/AIDS who face significant barriers to independent living. The program serves individuals ineligible for benefits from the HIV/AIDS Services Administration (HASA), often due to exceeding the income limits or not meeting the eligibility criteria for financial assistance. Units will be scattered primarily throughout Brooklyn in apartments that meet HUD criteria and are located near public transportation.

Position: Program Manager

Reports To: Senior Program Director

What the Program Manager Does:

  • What does the Program Manager do: 

    • Understand all aspects of contract requirements and communicate this understanding to staff. 
    • Train staff in program responsibilities, processes, and procedures, and ensure that all staff meets contract requirements in their daily work. 
    • Participate in the grant writing process and other funding initiatives.
    • Develop and maintain key community partnerships.
    • Oversee the development of social marketing campaigns. 
    • Work with staff to set realistic weekly and monthly performance targets in accordance with contract requirements. 
    • Coordinate and supervise all day-to-day activities of direct reporting staff. 
    • Periodically observe direct reporting staff engaging with clients and/or supervisory staff. 
    • Conduct weekly client progress reviews with staff. 
    • Conduct periodic internal file review and implement Quality Assurance measures as needed to ensure quality service delivery to clients. 
    • Provide crisis counseling to clients as needed.
    • Prepare contract reports and statistical information for both CAMBA management and funder use. 
    • Complete program forms and reports for the agency’s finance, payroll and human resources departments as required. 
    • Assist in the analysis of program and demographic data to make programmatic improvements. 
    • Organize onsite and offsite training opportunities for professional development of staff.

    • Communicate with peers on issues related to client progress and best program practices. 
    • Conduct regular staff meetings. 
    • While not on call, will be expected to be reachable after hours and on weekends to assist and consult on client emergencies when needed.
    • Confer with the Senior Program Director on programmatic and operational matters.
    • Attend and represent the organization at staff, funder, and external agency meetings as required. 
    • Review and sign program documentation completed by staff 
    • Review and sign time sheets. 
    • Prepare performance appraisals for direct reporting staff. 
    • Conduct initial intake or assessment of clients and/or clients’ families and periodic reassessments as needed. 
    • Prescreen clients over the telephone for eligibility and schedule intake appointments as needed. 
    • Plan, coordinate, and facilitate social/peer support events, including group facilitation for clients as needed. 
    • If in the event the program is understaffed, supervisors will take on the responsibilities and duties of the supervisees.
    • Other duties as assigned. 

Education, Licenses and/or Certifications Required: 

  • Bachelor’s degree (B.A.) and three years of applicable experience and/or equivalent experience.

Minimum Experiences and Skills Required:  

  • Bilingual English/Spanish 
  • Relevant experience as described above, preferably in a mid-sized (250+ employees) organization. 
  • Knowledge about, understanding of, and ability to work closely with persons Chronically Homeless, and or living with HIV/AIDS and related issues. 
  • Familiarity with housing in congregate/transitional settings 
  • Excellent written & verbal communication. 
  • Computer literacy in Microsoft Office Suite.

Compensation: $79,567 annually

Status: Full-time (35 hours per week)
